Customer must notify Professional Medical Warehouse of any discrepancies with their order within 24 hours of receipt. 

Professional Medical Warehouse will issue a Return Authorization for any incorrect product(s) shipped due to Professional Medical Warehouse’s error or product damaged in transit by courier.  Pictures may be required in the event of damaged product(s) prior to issuing a Return Authorization Number. 

Various RX pedigree products as well as special order products and custom ordered products are non-returnable, unless shipped in error by Professional Medical Warehouse.  These items will be noted as a non-returnable on packing slip and invoice. 

For returns of all RX products, a RX Return Authorization Form must be completed and returned to Professional Medical Warehouse prior to Professional Medical Warehouse issuing a Return Authorization Number. 

Any items returned 30 days past the invoice date must have written approval, will be subject to a 15% restocking fee plus applicable shipping charges and will be issued credit only, no refund. 

Products returned without notifying Professional Medical Warehouse will be returned to customer at their expense.
